**You can download the health form from our Forms page. 

• Please complete the entire enclosed health form. We require a new form each year. This protects both your child and the camp.
• Each camper is required to have been examined by a licensed physician within the past 24 months. Your doctor must complete and sign the required section of the health form. Return the form no later than two weeks before your child is to attend camp.
• All medications must be in the original containers. Prescription medications must have the physician’s dosage label attached. Non-prescription medications must be accompanied by a written note from the parent/guardian with dosage instructions.
• All medications will be dispensed by the camp nurse/first aid coordinator. Please pick up your child’s medications from the nurse/first aid coordinator before leaving camp.
• Children with varying degrees of ability are welcomed at Camp Grier. Please contact the Camp Director prior to your child’s camp date so that we are aware of any special needs and can plan accordingly.
• Dietary restrictions should be noted in writing and provided at registration. Please complete the food allergies form at the First Aid table when you bring your child to camp. Feel free to call ahead to discuss any concerns before coming to camp.
